📋 Description

• Plan and manage complex care coordination cases and activities • Support timely patient access, treatment progression, and continuity of care • Conduct assessments and support care planning • Facilitate transitions of care addressing clinical, social, and access barriers • Coordinate with providers, pharmacies, payers, and internal partners • Resolve access barriers, align next steps, and maintain case continuity • Analyze case trends, recurring issues, and workflow patterns to identify root causes • Recommend process improvements • Develop and refine job aids, care coordination templates, and documentation standards • Guide less experienced team members on complex case handling, escalation practices, and documentation expectations

🎯 Requirements

• Bachelor's degree in nursing, social work, health sciences, public health, healthcare administration, or a related field, or equivalent experience required • 4+ years of experience in care management, case management, clinical support, patient services, or a related field required • Certification in case management, care coordination, clinical specialty, or equivalent preferred
