⢠Collect, organize, record, and communicate data for primary health assessments and detailed medical histories ⢠Develop time-sensitive treatment plans for selected case-managed patients or populations ⢠Assess patients by telephone using established protocols ⢠Establish priorities for patient care and monitor and evaluate progress toward stated goals ⢠Provide coordinated, efficient, and effective health care to beneficiaries ⢠Oversee discharge-planning activities ⢠Assist health care finders in identifying patient referral needs ⢠Negotiate, write, finalize, and administer product/service agreements ⢠Apply utilization review criteria and report quality or risk-management issues ⢠Apply VA/DOD Clinical Practice Guidelines to standardize care ⢠Evaluate healthcare financial and population-health resource considerations ⢠Use research tools to evaluate practices, opportunities, and outcomes ⢠Manage information systems and technology for individual or population health and outcome communication ⢠Collect, track, trend, and analyze data using computers and software applications ⢠Design and present computer-generated outcome analyses, educational programs, and project briefings ⢠Apply pharmaceutical knowledge to evaluate effects, side effects, and complications ⢠Comply with KBR safety, quality assurance, attendance, training, and other company requirements ⢠Perform other assignments and duties as required
⢠U.S. Citizenship required ⢠Secret Clearance or Top Secret Clearance, or ability to obtain a clearance ⢠Current, full, active, and unrestricted license as a Registered Nurse or Licensed Clinical Social Worker in one of any U.S. State, the District of Columbia, Guam, Puerto Rico or U.S. Virgin Islands ⢠Minimum of two (2) years nursing experience or case management experience ⢠Must be certified as a case manager by a recognized certifying organization, or obtain certification within the first year of employment ⢠Demonstrated skill in understanding diagnoses ⢠Knowledge of patient education principles and management of complex medical, psychosocial, and financial problems ⢠Ability to seek feedback from peers, professional colleagues, clients, and outcomes research ⢠Skills to consult with health care providers and negotiate with outside providers ⢠May be required to receive and maintain clinical practice privileges ⢠Maintain credentialing requirements in good standing at a local MTF ⢠Fluent oral and written communication skills in English ⢠Skill in team coordination, facilitation, and communication ⢠Knowledge of local, state, and federal laws and regulations, professional code of ethics, and practice standards ⢠Must maintain current Basic Cardiac Life Support (BCLS) certification by the American Heart Association ⢠Compliance with safety, health, environmental, quality assurance, attendance, training, and other company requirements ⢠Previous military background/experience desired
