📋 Description

• Oversee monthly and quarterly ECM Report Template File and Outreach Transmission File submissions to contracted health plans • Gather, validate, and consolidate member data from multiple internal systems • Ensure submissions meet health plan formatting and regulatory requirements • Monitor submission timelines and troubleshoot errors or rejections • Conduct recurring member eligibility checks and reassess service enrollments with case managers • Document re-qualification activities and update member statuses • Manage initial authorizations and reauthorizations through provider portals • Track authorization approvals, denials, and pending renewals • Duplicate Lead Care Manager notes from TheraPlatform into SalesMate • Standardize note formatting and perform data integrity audits • Compile weekly member-status updates and leadership reports • Track trends in outreach, reassessment, engagement, service utilization, and eligibility • Review and update member profiles, discharge reasons, service end dates, and transitions • Support compliance, internal audits, and quality improvement initiatives • Research demographic, economic, healthcare utilization, regulatory, and community data for program expansion • Build databases of potential partners, community events, and outreach opportunities • Research and coordinate community events, conferences, and networking opportunities • Track post-event outcomes and engagement metrics • Assist with data implementations, migrations, workflow redesigns, audits, and compliance reviews • Help onboard and train staff on data procedures, system use, and reporting protocols • Work cross-functionally with IT to troubleshoot systems and optimize workflows

🎯 Requirements

• Bachelor’s degree in data management, healthcare administration, information systems, or related field (or equivalent experience) • Experience in healthcare data administration, reporting, or compliance strongly preferred • Proficiency with CRM platforms, reporting dashboards, and data entry tools • TheraPlatform and SalesMate familiarity is a plus • Strong analytical, organizational, and problem-solving skills • Ability to communicate clearly with executives, managers, and cross-functional teams • Familiarity with Medi-Cal, ECM, CalAIM, and health plan compliance processes preferred
