📋 Description

• Conduct scheduled community visits to care for patients in various settings • Care for patients of all ages with differing complexity of care needs • Provide ethical and safe nursing care within scope of practice • Assess patients’ individual needs using appropriate assessment tools and techniques • Execute, continuously monitor, and adjust care plans as required • Build rapport and trust with patients using a collaborative approach • Maintain and update professional patient records related to encounters and care plans • Provide required on-call coverage, including after hours

🎯 Requirements

• Registration with the CNO as a Registered Practical Nurse in good standing • Palliative care experience is mandatory • Post-graduate education in Palliative care is required • Valid driver’s license and access to a vehicle is required • Experience in delivering community care is an asset • Reliability and flexibility • Willingness to travel to and from patient residences within a designated area • Experience working with the vulnerable population • Passion for making a difference in your community • Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
