📋 Description

• Coordinate and deliver patient-care services as part of the Clinic Solutions Team • Act as a single point of contact for physicians, clinic nurses and patients • Provide customized administrative and clinical support for physicians and take ownership of patient onboarding • Coordinate care between HCPs, patient support programs and pharmacy • Interact with patients, insurance companies, provincial drug programs, pharmacies and other healthcare professionals to obtain medication coverage • Provide injection training and patient education on disease state and medication • Perform HCP and patient registration activities according to protocols • Process pertinent data-entry activities • Educate patients and HCPs on inflammatory bowel disease, advanced drug therapies and disease management • Assist physicians with patient support program paperwork and follow up on reimbursement or coverage submissions • Support physicians and patients with renewals and coverage changes • Counsel and educate patients receiving biologic and targeted IBD therapies on administration, adherence, monitoring and adverse effects • Facilitate communication among physicians, Patient Support Programs and healthcare professionals • Conduct chart reviews for special authorization forms • Coordinate medication fulfillment with Sentrex Pharmacy • Attend specialized training sessions for specific therapies • Provide in-person and virtual support, including adherence support • Review patient files and document treatment history • Maintain confidentiality of patient and corporate information • Administer IM and SC medications in a specialty clinic setting as required • Provide virtual oversight and education for home injections • Monitor patients post-injection and document completed activities • Report and document adverse events according to pharmacovigilance requirements • Adhere to professional nursing standards and Sentrex policies and procedures • Perform additional duties assigned by the manager

🎯 Requirements

• Minimum 2-4 years of clinical experience in GI/IBD (preferred) • Licensed RN or LPN in British Columbia • Liability Insurance required • CPR and First-Aid certification required • ACLS certificate required • Must hold a valid and unrestricted driver’s license • Able to confidently deliver patient self-injection education • Experience in critical care, emergency and acute care is a strong asset • Confident and experienced with Excel, Outlook and Word • Able to work autonomously with little supervision as part of a larger remote team • Able to travel within a set geographical boundary, ad hoc • Experience in Patient Support Programs is a strong asset • Experience in Neurology, Immunology, Oncology, Gastroenterology or Rheumatology is a strong asset • Understanding of the pharmaceutical industry is an asset • Strong analytical and mathematical skills • Effective interpersonal skills • Strong organizational skills and attention to detail • Ability to meet deadlines and be flexible with patient and HCP requirements • Excellent problem-solving skills; ability to resolve issues effectively and efficiently • Ability to communicate effectively both orally and in writing • Adhere to Sentrex Policies and Procedures
