• Leads and evolves the APC care delivery model across multi-state markets. • Partners with executive and clinical leadership to drive scalable innovation, influence strategic planning. • Oversees development and implementation of clinical standards, workflows, and policy frameworks. • Accountable for clinical OKRs and key enterprise KPIs, including clinical documentation accuracy (HCC), readmission rates, quality ratings (HEDIS), and patient satisfaction. • Ensures enterprise-wide clinical operations comply with CMS, Medicare Advantage, and applicable federal/state regulations. • Provides strategic leadership to APCs and management teams across multi-state markets. • Serves as a key clinical advisor and connector across virtual care, case management, IPAs, and external partners.
• 10+ years of experience as a Nurse Practitioner or Physician Assistant. • 3+ years experience leading clinical staff, preferably within a high-risk clinic and/or home-based care delivery environment. • Master’s Degree NP or PA license with active board certification • Knowledge of CPT, ICD-9 and HCPC codes • Effective written and oral communication skills; ability to establish and maintain a constructive relationship with diverse members, management, employees and vendors. • Current unrestricted state license and current state driver’s license; Nurse Practitioner and/or Physician Assistance Board Certification; Medicare Part D Prescriber; NP or PA license with active board certification NP (Nurse Practitioner): Active RN License (Must, upon hire) Active NP License (Must, upon hire) Furnishing number (Must, upon hire) NPI Number (Must, upon start) DEA Number (Must, within 6 months of start) Board Certification: AANP, ANCC (Must, within 6 months of start) Valid BLS (Basic Life Support) (Must, upon start) PA (Physician Assistant): Active PA License (Must, upon hire) NPI Number (Must, upon start) DEA Number (Must, within 6 months of start) Board Certification: NCCPA (Must, within 6 months of start) Valid BLS (Basic Life Support) (Must, upon start)
