📋 Description
• Responsible for health care management and coordination within the scope of licensure • Assessing, developing, implementing, coordinating, monitoring, and evaluating care plans • Coordinates and monitors member’s progress and services • Interfaces with Primary Care Physicians, Hospitalists, Nurse Practitioners, and specialists
🎯 Requirements
• Minimum (5) years' clinical case management experience • Bachelor's degree in Nursing • Current, unrestricted RN license in California • Problem-Solving Skills: Effective problem solving, organizational and time management skills
