📋 Description

• Help NDIS participants understand their plans • Connect participants with appropriate supports • Help participants build choice, confidence, and independence • Manage a caseload and independently organise workload • Build trusted relationships with participants and community stakeholders • Identify and implement local solutions • Follow participant support activities through to completion • Work within MyIntegra’s Support Coordination group as part of the APM Group

🎯 Requirements

• Relevant experience in disability, mental health, community services, allied health, case management, or another relevant human services setting • Ability to manage own workload • Strong communication and relationship-building skills • Ability to organise work, manage a caseload, and follow tasks through • Knowledge of the local community and ability to find local solutions • Willingness to undertake further study and continue professional development if needed • A relevant qualification is welcomed but not required • Commitment to supporting NDIS participants
