📋 Description

• Support NDIS participants in navigating their plans • Connect participants with appropriate supports • Work through barriers and help participants build choice, confidence, and independence • Build trusted relationships with participants, families, providers, and other stakeholders • Coordinate stakeholders around participant goals • Manage different support needs, competing priorities, and multiple stakeholders • Find practical solutions for participant-specific barriers • Manage and prioritise a caseload • Follow participant support matters through as circumstances change

🎯 Requirements

• Demonstrated Support Coordination experience • Strong understanding of the NDIS • Knowledge of local communities and service landscapes • Strong networks and relationship-building with participants, families, providers, and stakeholders • Ability to manage complex support needs, competing priorities, and multiple stakeholders • Resourceful and practical problem-solving skills • Ability to confidently manage and prioritise a caseload and follow tasks through • Relevant qualification (Cert III or above) in disability, mental health, allied health, community services, or another human services discipline is welcomed • Experience supporting participants with more complex needs is welcomed
