📋 Description
• Provide virtual-first psychosocial assessments and skill-based psychotherapy to individuals with serious mental illness • Facilitate patient engagement and follow-up care • Provide audio-only follow-up sessions in the event of technical issues • Document treatment response and monitor clinical symptoms • Provide support and advocacy to caregivers and family members • Lead/facilitate support groups and group therapy • Identify social determinants of health needs that team members will work to resolve • Participate in huddles and multidisciplinary communication
🎯 Requirements
• Experience with screening for common mental health and/or substance use disorders. • Experience with assessment and treatment planning for common mental health and/or substance use disorders. • Experience conducting psychotherapy via telehealth • Active and unrestricted licensure as one of the following in Florida (or ability to become licensed in Florida): Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC) • Demonstrated ability to collaborate and communicate effectively in a team setting • Ability to maintain effective and professional relationships with patients and other members of the care team • Comfort working in a startup environment with a rapidly growing and evolving team
