• Empower members by supporting their health and well-being • Assist members in finding and maintaining competitive, integrated employment • Navigate challenging relationships to ensure members receive necessary services • Perform intake, enrollment, and ongoing assessments • Develop and maintain Person-Centered Plans
🎯 Requirements
• RN or B.S. in social work, nursing, education, human service or health care profession • Certification from the Developmental Disabilities Nurses Association for RNs or LPNs preferred • 5+ years’ experience as an independent Support Coordinator or case manager in a 1915(c) HCBS Waiver in lieu of a degree • 2 years of experience in the healthcare industry or operations type environment required • Valid driver's license • Digital literacy assessment
