📋 Description
• Serve as the frontline support for seniors navigating social and clinical situations. • Remove barriers to help patients reach the best health outcomes. • Address social determinants of health (SDOH) and coordinate various assistance resources. • Manage and guide patients through chronic conditions by improving health literacy. • Build long-term, trusting relationships with patients to ensure they achieve their healthcare goals. • Collaborate with physicians to develop individualized care plans.
🎯 Requirements
• Active Compact or California Registered Nurse (RN) license in good standing. • Deep understanding of social determinants of health (SDOH) and experience working with diverse, vulnerable patient populations. • Exceptional active listening and interpersonal skills. • Strong administrative abilities and clear communication. • Comfortable learning a new EHR, adopting AI tools, and picking up evolving operational processes quickly. • Resilience, persistence, and creativity to navigate barriers in the US healthcare system. • Preferred: Background in community health, health coaching, or experience with Medicare populations. • Preferred: Fluency in Spanish or other languages.
