📋 Description
• Develops, assesses, and facilitates complex care management activities • Evaluates the needs of the member and recommends appropriate care • Develops ongoing care plans and collaborates with providers • Identifies problems and provides care management interventions • Coordinates care between members, caregivers, and care provider teams • Provides ongoing follow-up and monitoring of member status
🎯 Requirements
• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing • 2 – 4 years of related experience • RN - Registered Nurse - State Licensure and/or Compact State Licensure required
