📋 Description

• Triage patients using protocols to determine what measures can be taken by the nurse and when to escalate to appropriate level of care. • Provide a virtual nursing assessment to evaluate patient symptoms, medication adherence, and disease progression. • Provide patient education on heart failure medications, adherence, and potential side effects to patients and caregivers • Monitor patient vital signs and survey responses, identify changes in condition and document. • Document interactions and interventions. • Answer, direct, and follow up on incoming calls. • Recognize urgent or emergent situations, seek assistance of the medical provider, and initiate appropriate emergency interventions as directed.

🎯 Requirements

• Bachelor's Degree in Nursing, required. • Current, active, unrestricted license to practice as a Registered Professional Nurse in a compact state. Must be in good standing with the Board of Registered Nurses • Minimum of 3 years experience in cardiology, heart failure management, or critical care nursing. • Strong background in heart failure patient management, including knowledge of GDMT (Guideline-Directed Medical Therapy). • Advanced certification highly preferred (CCRN, CHFN, RN-BC) • Experience with patient phone triage either in-office or via telemedicine. • Ability to learn and proficiently use multiple EMRs and various communication applications, including Google products. • Must be comfortable with the virtual use of episodic vital sign monitoring devices. • Ability to work from home in a HIPAA compliant environment is required. • Fluency in English and Spanish is highly preferred. • Some holidays may be required.
