📋 Description

• Enable equitable, coordinated care by strengthening community and provider ecosystems • Improve care navigation and equip the workforce to deliver integrated, culturally responsive care • Build and steward trusted relationships with communities, providers, and partners • Identify and build opportunities to increase member and provider engagement • Create models for and execute on opportunities to enable community workforce to deliver integrated, culturally responsive care • Ensures enterprise colleagues and community workforce have the necessary skills, tools, and confidence to support equitable care navigation (training, education, etc.)

🎯 Requirements

• 8-10 years of prior relevant work experience • Population health program implementations • Demonstrated experience in building interventions to address upstream barriers for better health outcomes • Prominent level of interpersonal skills and ability to manage multiple business areas, community and provider partners • Coordinates and prepare for external community partners and provider meetings • Ensures that the most impactful internal subject matter experts (clinical, regulatory, financial, analytical, etc.) are utilized to optimize performance • Experience developing analyses and presentations to explain business issues, quantify financial impact, and provide recommendations • Exceptional oral, written, and presentation communication skills • Attention to detail and proactive problem‑solving • Proven ability to interact with, influence and collaborate with internal and external stakeholders at all levels • Experience managing matrixed environment with ability to leverage internal business partners to complete intervention design
