📋 Description

• Leading a team of IKC RNs who case manage dialysis and CKD patients • Supporting a team of non-clinical IKC care coordinators • Managing team performance, productivity, and creating a special place to come to work • Representing the IKC leadership team with other IKC leaders • Effectively managing Disease Management programs in designated market or geography • Reviewing and analyzing data to drive patient care outcomes in programs across the market • Building and managing effective relationships with internal and external clients • Providing clinical support to reporting Registered Nurse Case Managers • Managing all teammate employment/HR activity such as payroll, PTO, hiring, onboarding duties, and professional development • Ensuring compliance to all applicable regulations and DaVita policies • Monitoring risk management and quality assurance compliance • Achieving quality and metric goals in Integrated Kidney Care programs • Coordinating activities with DaVita IKC management team

🎯 Requirements

• Minimum of 3-5 years’ management experience leading and managing teams required • Demonstrates experience managing to data driven outcomes and process improvement required • Strong communication on medical management, operational, and business-related issues • Minimum of 1-2 years’ experience in clinical nursing leadership highly preferred • RN license preferred • Current CPR certification required • Intermediate computer skills and proficiency in MS Word, Excel, PowerPoint, and Outlook required • Home office with Internet connectivity at minimum of 1MB upload and 1MB download speed required
