📋 Description

• Organize and conduct consultation appointments within the facility • Maintain a self-reflective, responsible and respectful counseling approach • Document counseling processes and patients' expressed wishes (advance directives) and communicate this information to care and support teams • Raise staff awareness to responsibly ensure the implementation of expressed wishes • Establish structured networking and collaborate with regional healthcare providers involved in care planning

🎯 Requirements

• Completed vocational training as a qualified nursing professional or a relevant degree in a health-related field • Three years of professional experience within the last eight years in inpatient or outpatient care facilities or in a hospice • Further training/certification as a consultant in advance care planning for the last phase of life in accordance with §132g SGB V • Strong interpersonal skills and empathy
