📋 Description
• Provide tiered after-hours phone support • Clinical guidance and direction to clients and community partners • Support systemic coordination of information after hours • Enhance care provision through monitoring workflow and assisting direct care staff • Perform telehealth visits for immediate virtual nursing assistance • Promote continuity of care through real time documentation • Collaborate and communicate effectively
🎯 Requirements
• Graduate of approved nursing program • Current unrestricted Florida RN license • Valid and current CPR certification • Minimum one year experience in an acute-care Hospital environment providing direct patient care • Previous experience as a Hospice RN preferred • CHPN certification strongly encouraged within three years of employment • BSN or MSN preferred • Equivalent combination of professional experience and education may be considered
