📋 Description

• Provide direct support to patients and families in addressing social needs • Work collaboratively with community partners and care team members to close gaps related to non-medical drivers of health • Assist in building and strengthening community relationships • Identify resources and community support available to the family or caregivers • Provide information, referrals, and non-clinical assistance with linkage to medical, psychosocial, and basic needs services • Empower families to advocate for themselves by providing education, guidance, and supportive tools to navigate systems • Assist with accessing specific programs and supports to address psycho-social needs both within Imagine Pediatrics and in the larger community • Advocate with community agencies for, and on behalf, of clients and families to decrease service gaps and remove barriers to services • Connect with relevant internal staff and providers to assist with system navigation • Assist in development of an internal community resource library

🎯 Requirements

• Bachelor’s degree or higher in healthcare, behavioral health, or social services related field preferred • Certification as a Community Health Worker highly desired • Familiar with navigating social service agencies • Must have reliable transportation, a valid Driver's License, and ability to produce a copy of current auto insurance certificate • Experience with resourcing and telephonic services • Bilingual Spanish required
