📋 Description

• Conduct comprehensive reviews of patient care documentation according to established standards, protocols, and regulatory requirements • Complete and submit accurate Minimum Data Set (MDS) assessments within required timelines • Develop and update individualized care plans for residents • Ensure assessments and documentation align with Medicare/Medicaid, CMS, and state regulations • Collaborate with nurses, therapists, clinical staff, managers, and other healthcare stakeholders to support quality patient care • Maintain precise documentation to support reimbursement and compliance • Apply knowledge of PDPM (Patient-Driven Payment Model) to support accurate reimbursement classification • Review patient records and clinical documentation for completeness and accuracy • Identify missing, incomplete, or incorrect information and follow up accordingly • Provide feedback and support ongoing education and training based on documentation review findings • Manage multiple deadlines independently in a remote work environment

🎯 Requirements

• Current and valid PHRN license • Must have bedside/clinical experience • Must have at least 2 years experience with MDS (Minimum Data Set) • Extensive team lead / supervisory experience • Experience supporting a U.S. Skilled Nursing Facility (SNF) • Strong knowledge of prehospital patient care protocols, standards, and regulatory requirements • Excellent attention to detail and analytical skills • Effective communication and interpersonal abilities • Ability to work independently and collaboratively in a team-oriented environment • Commitment to continuous learning and professional development • Willingness to work on a shifting schedule
