📋 Description

• Conduct comprehensive reviews of patient care documentation according to established standards, protocols, and regulatory requirements • Complete and submit accurate Minimum Data Set (MDS) assessments • Develop and update individualized care plans for residents • Ensure assessments align with Medicare/Medicaid and state regulations • Collaborate with nurses, therapists, clinical staff, managers, and other healthcare stakeholders • Identify areas for improvement and provide feedback for ongoing education and training • Maintain precise documentation to support reimbursement and compliance

🎯 Requirements

• Current and valid PHRN license • At least 1 year of bedside/clinical experience • Experience with MDS (Minimum Data Set) • Strong knowledge of prehospital patient care protocols, standards, and regulatory requirements • Excellent attention to detail and analytical skills • Effective communication and interpersonal abilities • Ability to work independently and collaboratively in a team-oriented environment • Commitment to continuous learning and professional development • Must be residing within GMA (Greater Manila Area) • Willing to work on shifting schedule
