📋 Description
• Gather pertinent information from incoming calls concerning medications • Address medication related questions, issues, prior authorizations, as well as refill approvals • Perform medication review during the call and complete clinical review • Enter into a collaborative practice agreement with health system physicians • Review patient medication profiles to guard against drug sensitivities, contraindications, duplications, and major drug interactions • Document encounters within electronic medical record and e-scribe approved prescription refills
🎯 Requirements
• Bachelor's Degree in Pharmacy (Required) • Doctoral Degree (Preferred) • Minimum of 4 years-Related work experience (Required) • Licensed Pharmacist (Pennsylvania) within 90 days
