📋 Description

• Contact caregivers prior to or during scheduled visits to confirm arrival at the patient’s home. • Ensure caregivers have the current and appropriate patient Plan of Care. • Email or securely transmit care plans and related documentation to caregivers when necessary. • Document all communication, follow-ups, and care coordination activities accurately in the EMR system. • Escalate patient care concerns, missed visits, or compliance issues to the appropriate clinical supervisor. • Maintain HIPAA compliance and patient confidentiality at all times. • Support continuity of care and timely coordination between field staff and office personnel. • Assist with basic care coordination and administrative nursing tasks as assigned.

🎯 Requirements

• Registered Nurse with New York License - USRN - NY • Fluency in both Spanish and English required • Minimum 1–2 years of experience in home health, care coordination, or clinical scheduling preferred • Strong organizational and time-management skills • Proficiency with scheduling software and electronic health records (EHR) systems • Excellent interpersonal, problem-solving, and multitasking abilities
