📋 Description

• Visits with persons on the caseload and attending team meetings • Completing or attend meetings on needs assessment; individual service plan development (CSSP) • Assisting with planning for new service development • Locating residential, vocational, and other needed services depending on each person’s needs • Monitoring service delivery and ensuring the health and safety needs of each person are being met. • Completion of referrals and crisis intervention as needed • Developing waiver budgets • Completing case notes • Acting as an advocate for people we provide services to

🎯 Requirements

• Be a licensed Social Worker (Bachelor of Social Work required for licensure) • Or licensed Graduate Social Worker (Master’s in Social Work required for licensure) as stated by the Minnesota Board of Social Work • Or have a 4-year degree in Human Services, Psychology or Sociology, or related fields • Successful clearance of Department of Human Services background check • Successful clearance of Motor Vehicle Background Check and acceptable driving record per Company Policy
