📋 Description
• Enhance continuity of patient care by providing collaboration between hospital, physicians, hospice/home care, Home Medical Equipment Services, and patients and caregivers. • Facilitate patient transition to post-acute care settings. • Build and maintain partnerships enhancing post-acute care referrals to Alliance. • Collaborate with health care teams to improve service offerings and implement initiatives supporting better patient care.
🎯 Requirements
• Registered Nurse • 3-5 years of experience in nursing • Associates Degree in Nursing (Required) • Bachelor's Degree in Nursing (Preferred) • Certification in clinical specialty (Preferred) • Valid Registered Nurse license in Tennessee • BASIC LIFE SUPPORT certification from American Heart Association
