📋 Description

• Support implementation of the Hemoglobinopathies project in Umaria district • Mobilize communities and facilitate awareness, counselling, and screening for Sickle Cell Disease and Thalassemia • Strengthen linkages between community members and public health systems • Build capacity of community health workers on SCD and Thalassemia awareness, counselling, and screening • Assist with community-based screening activities and mobilize eligible populations • Organize awareness sessions, school health campaigns, premarital counselling, and outreach programs • Counsel individuals and families on disease management, prevention of complications, and healthy lifestyles • Support distribution and appropriate use of IEC/BCC materials • Form and facilitate Patient Support Groups at community and facility levels • Coordinate referrals and follow-ups between communities and health facilities • Work with ASHAs, ANMs, and CHOs on community tracking and reporting • Collect and document screening, referral, counselling, and Patient Support Group data • Participate in district- and block-level review meetings and share field updates • Conduct regular field visits to ensure service quality and community engagement • Submit monthly outreach plans, activity reports, event documentation, and field feedback

🎯 Requirements

• Bachelor's degree in Social Work, Psychology, Public Health, Nursing, or a related field is required • At least 2–3 years of experience in community outreach, public health programs, or patient counselling, preferably in rural or tribal areas • Experience with genetic or chronic conditions such as thalassemia or sickle cell disease is desirable • Familiarity with government health systems, including ASHAs, ANMs, PHCs, and CHCs, is preferred • Skills in community mobilization, coordination, and organizing health camps and awareness activities • Ability to provide effective health counselling and psychosocial support, particularly for individuals with chronic diseases • Ability to support training of frontline health workers and facilitate small-group education sessions • Ability to coordinate with PHCs, CHCs, and district hospitals for service delivery and patient follow-up • Basic skills in data collection, documentation, patient records, and simple reports • Up to 60% field travel within assigned blocks and health facilities in the district
