📋 Description
• Deliver comprehensive care management services across diverse patient populations • Conduct patient assessments and develop individualized care plans • Support chronic disease management (e.g., diabetes, hypertension, CHF, COPD) • Provide patient education on conditions, medications, and self-management strategies • Use motivational interviewing techniques to improve adherence and engagement • Coordinate care with providers, pharmacists, and other healthcare professionals • Identify care gaps, risks, and barriers to treatment adherence • Ensure accurate and timely documentation in compliance with program standards • Support care transitions and continuity across settings
🎯 Requirements
• Active Registered Nurse (RN) Compact License (NLC) • Minimum of 2 years’ experience in care management, case management, or chronic disease management • Willingness to work across multiple state populations
