📋 Description
• Process and manage member referrals received from the Marketing Department • Educate prospective members and families on home care service requirements • Coordinate assessments and assist with the enrollment process • Maintain ongoing communication with members and families throughout enrollment • Serve as a liaison between members, nursing facilities, physician offices, hospitals, and insurance companies • Follow up on required documentation and enrollment requirements • Update daily reports and maintain accurate records in the system • Ensure all member information is accurately documented and maintained
🎯 Requirements
• At least 1 year of experience as an Intake Specialist, Home Care Intake Coordinator, or Long-Term Care Liaison • Fluent in English and Spanish • Strong customer service and communication skills • Compassionate, professional, and solution-oriented • Strong follow-up and organizational skills • Detail-oriented with accurate documentation abilities • Ability to manage multiple referrals and enrollment cases simultaneously • Comfortable communicating with members, families, healthcare providers, and insurance companies
