📋 Description

• Conduct virtual wellness visits directly to patients in their homes • Perform virtual adult and/or pediatric wellness visits, health assessments, and more • Engaging in proactive care by conducting thorough health risk assessments, medication reviews, cognitive screenings • Collaborate with Sprinters to evaluate vital signs, arrange blood draws, and provide comprehensive care coordination and management

🎯 Requirements

• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner License in California • Credentialed Medicaid provider for the state of California (Medi-Cal) • Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking • Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination • Strong written and verbal communication skills • Ability to work independently or in a team environment • Pass national background check and valid clinical license search
