📋 Description

• Locate residential and other provider agencies for members enrolled in Tailored Care Management. • Work closely with the Primary Care Manager and Regional Care Teams to locate appropriate residential and other supports based on member’s level of care needs. • Complete required documentation/paperwork/tasks in a software platform according to required timelines. • Assist with integrated care management through multi-disciplinary collaborations and referrals.

🎯 Requirements

• High School Diploma or GED and a minimum of one (1) year working experience focused on working with individuals with Intellectual Developmental Disabilities (IDD) or Traumatic Brain Injury (TBI), or BH/MH/SU, or Innovations populations; OR Equivalent combination of education/experience. • Must have a valid driver’s license. • Must reside within Trillium’s catchment area which includes North Carolina counties: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, Warren, Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, Wilson, Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, Wayne, Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, Scotland, Anson, Guilford, Montgomery, Randolph, and Richmond. • Must be able to travel within catchment as required.
