• Managing the continuum of care for admission through discharge for assigned patients • Reflecting appropriate knowledge of the RN scope of practice and current state requirements • Partnering with the multi-disciplinary team and utilizing scientific evidence for best practices • Coordinating the discharge planning process in collaboration with the interdisciplinary team • Documenting all communication and concerns pertaining to coordinating care and services • Communicating with physicians regarding level of care or admission status when criteria are not met • Identifying patients/families with psychosocial and/or non-medical discharge planning needs • Assisting in Performance Improvement, Evidenced Based Practice/Research process
• Baccalaureate Degree in Nursing, ADN, or Diploma RN Degree • Three (3) years RN experience with current BLS certification • Valid Arkansas Registered Nursing License • National Nursing Certification preferred; BSN preferred; Certified Case Manager (CCM) or American Case Management (ACM) certification
