📋 Description

• Responsible for the provision of pharmacist care by working in collaboration with multidisciplinary healthcare teams to ensure quality care, education and research, and patient safety. • Works to promote guidelines and policies and assists in the development of programmatic changes to improve the care provided to patients throughout the health system. • Provides education to patients, providers, students, residents, and others. • Assumes a leadership role in the professions of pharmacy and quality improvement.

🎯 Requirements

• Must possess current licensure as required by state board where services will be provided: MD: Licensed Pharmacist through Maryland Board of Pharmacy OH: Licensed Pharmacist through Ohio Board of Pharmacy PA: Licensed Pharmacist through Pennsylvania Board of Pharmacy WV: Licensed Pharmacist through West Virginia Board of Pharmacy • Advanced degree (Master of Public Health or other degree) • Residency training OR three (3) years of pharmacist experience
