Beschreibung

• Provide key support to Care Managers in delivering appropriate services to members • Deliver most Tailored Care Management member-required contacts • Perform supportive outreach, follow-up, and engagement with members • Meet with members virtually or in person • Record member health updates • Suggest changes to Care Managers regarding current care plan efficacy • Help members find additional medical or community-based resources • Facilitate programs and activities promoting holistic health and improving members’ health literacy and education • Teach self-management skills and share self-help recovery resources • Distribute educational materials on chronic conditions • Arrange and schedule member services, appointments, wellness reminders, and non-emergency medical transportation • Collaborate with Care Managers and other care-team personnel regarding members’ care, health updates, and changes • Participate in relevant educational case conferences • Ensure proper storage and transmission of sensitive health records • Work from the remote location between 8:30 a.m. and 5:00 p.m. and travel within the community as needed

🎯 Anforderungen

• High school diploma or GED and one (1) year of qualifying experience through one of the listed alternative pathways • One (1) year of lived experience with an intellectual/developmental disability (I/DD) or traumatic brain injury (TBI), with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system; OR • One (1) year of lived experience with a behavioral health condition and certification as a Peer Support Specialist; OR • One (1) year of direct experience as a parent or guardian providing care for an individual with an I/DD, TBI, or behavioral health condition and navigating the Medicaid delivery system; OR • One (1) year of paid experience performing the described essential duties, including at least one (1) year working directly with the Tailored Care Management eligible population • Valid driver’s license • Certified Peer Support Specialist if used as a qualifying pathway • Must reside within Trillium’s Catchment of North Carolina • Must be able to travel within the catchment as required • All candidates must pass a drug test as a condition of employment • Resume must include employer names, dates of service, average weekly hours, essential job duties, education details, and applicable licensure/certification information
