📋 Description

• Provide high-quality, evidence-based cardiovascular care through a dedicated digital platform for established patients • Conduct comprehensive virtual follow-up visits for cardiology patients using approved telehealth platforms • Provide care across General Cardiology, Electrophysiology, and Heart Failure • Review patient symptoms, interval history, medication adherence, and clinical progress • Evaluate laboratory results, cardiac imaging, rhythm monitoring, stress testing, echocardiograms, and other cardiovascular diagnostic studies • Support optimization of medical therapy and coordinate ongoing management • Determine appropriate follow-up intervals and coordinate future virtual or in-person appointments based on clinical needs • Review incoming cardiology consultation requests from internal and external referral sources • Collaborate with Heart Center Communications to support timely scheduling and appropriate patient placement • Facilitate communication between referring providers and cardiology specialists when additional clinical guidance is needed • Provide patient education regarding diagnoses, medications, lifestyle modifications, and disease management • Perform structured morning chart review, including outstanding diagnostic testing and recent hospitalizations or emergency department visits • Facilitate appropriate escalation of care when needed • Work closely with cardiologists, nursing staff, and other multidisciplinary team members to support timely, patient-centered care

🎯 Requirements

• Nurse Practitioners: Completion of an accredited Nurse Practitioner program with a Master's degree in Nursing • Physician Assistants: Completion of an accredited Physician Assistant program • Physician Assistants: Bachelor's degree required; Master's degree strongly preferred • Nurse Practitioners: National certification as an NP required prior to or within 6 months of employment • Nurse Practitioners: BLS required • Physician Assistants: Certification by the National Commission on Certification of Physician Assistants (NCCPA) required • Nurse Practitioners: Current or compact RN licensure in North Carolina • Physician Assistants: Licensure in North Carolina • Nurse Practitioners: 3 years of RN experience strongly preferred • Nurse Practitioners: Minimum of 1 year of cardiovascular clinical experience preferred • Physician Assistants: No experience required beyond education/training requirement, or equivalent combination of relevant education and/or experience • Ability to provide evidence-based cardiovascular care through a dedicated virtual platform • Ability to work with cardiologists, Heart Center Communications, nursing staff, and multidisciplinary team members
