📋 Description

• Counsel clients or patients individually and/or in family sessions • Accept referrals and maintain a client load • Make internal referrals to IOP when there is little to no improvement within 8 weeks of outpatient therapy • Guide clients in developing skills and strategies to address their problems • Develop and implement treatment plans with SMART goals within 72 hours of intake • Update and modify treatment plans every 4 weeks • Complete and maintain diagnostic records, progress notes, treatment records, and reports • Complete progress notes within 48 hours after every session • Evaluate clients' physical or mental condition based on client information • Collaborate with staff on clinical assessments and treatment plans within 2 days of establishing care • Refer patients, clients, or family members to community resources or specialists as needed • Maintain professional knowledge through literature, courses, seminars, and contact with social service agencies

🎯 Requirements

• Master's Degree from an accredited college or university • Two years of professional-level experience in a counselor setting • Active Licensed Professional Counselor license or Licensed Clinical Social Worker license in Virginia • Minimum client load of 18 hours per week • Preferred: 2+ years of healthcare experience with oncology • Preferred: 1 year of experience in telehealth settings
