📋 Description

• Assess patients’ medical and mental health needs and provide proactive plans of care • Address barriers to care and manage acute and chronic conditions • Ensure compliance with local, state, and federal agencies related to clinical services • Prescribe medications and order lab work, diagnostic procedures, and consultations • Monitor patients’ treatment compliance and response, modifying treatment plans as needed • Work with RN case managers as part of an integrated care team • Collaborate with providers, case managers, facility partners, and health plan leadership • Implement TruHealth’s national model of care in compliance with federal and state regulations • Provide training and advice to facility staff when required

🎯 Requirements

• FNP, AGNP, AHACNP or PA license required • Degree from an accredited APRN or PA program • 3 years’ experience in clinical nursing or rehab in geriatric populations • Electronic Health Records experience • Working knowledge of Microsoft applications, including Word, Outlook and Excel • Experience in primary care or internal medicine preferred, preferably with geriatrics focus • Advanced practice registered nurse (APRN or NP) or physician assistant (PA) qualification • Primary Care in Geriatric Medicine experience preferred • Long Term Care experience preferred • APRN Advanced Practice RN certification/license required
