📋 Description

• Assess patients’ medical and mental health needs • Provide plans of care that proactively manage conditions and address barriers to care • Ensure compliance with local, state, and federal agencies related to clinical services • Prescribe medications • Order lab work, diagnostic procedures, and consultations • Monitor patients’ compliance and response to treatment • Modify treatment plans as needed • Work with RN case managers as part of an integrated care team • Collaborate with providers, case managers, facility partners, and health plan leadership • Implement TruHealth’s national model of care in compliance with federal and state regulations • Exercise independent judgment to treat acute and chronic conditions, consulting with a supervising physician as appropriate • Provide training and advice to facility staff when required

🎯 Requirements

• FNP, AGNP, AHACNP or PA license required • Degree from an accredited APRN or PA program • 3 years’ experience in clinical nursing or rehab in geriatric populations • Electronic Health Records experience • Working knowledge of Microsoft applications, including Word, Outlook and Excel • Experience in primary care or internal medicine preferred, preferably with geriatrics focus • Experience with primary care in geriatric medicine preferred • Experience in long-term care preferred • May be required to provide training and advice to facility staff
