📋 Description

• Deliver patient care services through a remote technology platform • Provide holistic, evidence-based care, including assessment, diagnosis, treatment, health-problem management, health counseling, and disposition planning • Provide education and treatment for pregnancy prevention, STI prevention, and safer sex practices • Evaluate primary care, acute, chronically ill, and transitional care patients aged 18 months and above • Provide patient counseling on pregnancy prevention, STI prevention, safer sex practices, contraceptive care, and medication management • Educate patients on health maintenance and respond to patient care inquiries • Document all patient care in Epic/EHR according to MinuteClinic policies and procedures • Coordinate patient care with internal and external colleagues, including the broader patient-centered medical home • Work within a patient care team consisting of Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare team members • Work independently, prioritize and solve problems, take initiative, and advocate for patients and the practice • Work PRN evening shifts from 2:30p to 11p

🎯 Requirements

• Current, unrestricted licensure in FL, MA, and OH; qualified candidates must hold current licensure in the listed states and be willing and able to obtain additional licensure if hired • Candidates must be currently licensed in one of: D.C., DE, FL, KS, KY, MA, MD, ME, NH, OH, RI, VT • Minimum of two years of medically-relevant experience or equivalent • Completion of a Master’s Degree level Family Nurse Practitioner program • Current National Board Certification • State of Employment license to practice in the Advanced Practice Nurse role • Effective verbal, written, and electronic communication skills • Outstanding organizational skills and ability to multi-task • Initiative, problem solving ability, adaptability and flexibility • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology • Capacity to collaborate with professional colleagues • Depending on the market, proficiency in speaking and writing additional languages, including Spanish, may be required • Qualified candidates must have 20+ hours of availability per week • Broadband connectivity, a quiet setting with a neutral background, and ability to uphold patient privacy per CVSH guidelines • Ability to view and read computer screens and electronic devices; operate computer and phone; speak intelligibly and hear patient responses
