📋 Description

• Provide direct telemedicine care to patients with advanced chronic lung and heart disease • Perform telemedicine visits and provide condition-specific patient education • Assess and triage adult outpatients with multiple chronic medical conditions • Coordinate care with primary care providers, specialists, and external care agencies • Advocate for patients by activating community resources for medications, transportation, nutrition, and housing • Serve as a continuous point of contact for patients and families across transitional care, stabilization, rehabilitation, advanced care planning, and end-of-life care • Collaborate with nurse practitioners, physicians, medical directors, respiratory therapists, engineers, designers, and other interdisciplinary team members • Optimize patient care and contribute ideas for care improvement • Promote the program and answer questions from referring providers and practices • Report to the team lead nurse practitioner • Lead patients in home-bound exercises over telehealth visits

🎯 Requirements

• Completion of an accredited nursing program with a minimum of an Associate's Degree • Current or compact unrestricted nursing license in Virginia or Pennsylvania • 5+ years of experience caring for adult patients with medical illness across acute and chronic settings • Strong foundation in Primary Care, Internal Medicine, Geriatric, and Urgent Care practice environments • Strong triage skills • 1+ years of experience caring for adult outpatients with chronic cardiopulmonary conditions, including COPD, congestive heart failure, and coronary artery disease • Maintenance of license and continuing nursing education • Reliable high-speed internet connection of at least 25 Mbps • Ability to provide telemedicine and remote care • Experience or interest in chronic cardiopulmonary disease management, telemedicine, cardiac or pulmonary rehabilitation, and coordination of health services • Ability to lead home-bound exercises over telehealth visits • Strong interest in improving patients' lives and ability to work as a self-directed team player
