📋 Description

• Conduct regular check-ins with the assigned patient panel via telephone • Develop and maintain comprehensive care plans • Coordinate care between healthcare providers and services • Document all patient interactions in EHR system • Monitor and track patient progress and compliance • Identify and address barriers to care • Provide patient education on chronic condition management • Meet monthly performance metrics and quality goals

🎯 Requirements

• Medical Assistant certification strongly preferred; LPN and RN also accepted • Minimum 3 years of clinical experience in a healthcare or healthcare office setting • Strong knowledge of chronic conditions and care management • Proficiency in Electronic Health Records (EHR) systems and familiarity with clinical workflows • Excellent communication and interpersonal skills • Experience in patient education and care coordination • Strong problem-solving skills and the ability to operate independently
