📋 Description

• Research and verify services within the electronic medical record (EMR) • Ensure accurate documentation and identify when services began • Conduct outreach calls to members following inpatient hospitalizations or emergency room visits • Help coordinate members’ ongoing care needs • Facilitate warm handoffs and referrals to RN Care Managers • Collaborate with members, providers, and internal teams to promote continuity of care and timely access to services • Research concerns and provide resolutions to patient issues and needs • Collect and document client data in the medical record • Coordinate multiple appointments using scheduling systems • Identify care gaps and coordinate efforts to close gaps and issues • Follow up with members regarding needs and obstacles, health literacy, cultural issues, and social-class barriers • Meet and accompany patients and families to initial provider appointments before and after acute and post-acute transition • Establish collaborative relationships with physicians, payers, and healthcare team members • Collect and communicate pertinent information to fulfill utilization and regulatory requirements • Educate internal healthcare team members on care management and managed care concepts • Work independently to provide excellent patient care

🎯 Requirements

• Must reside in Arizona • High school diploma/GED or equivalent working knowledge • Knowledge of medical terminology • Completion of Medical Assistant Training, Patient Care Technician Training, or Certified Nursing Assistant Training • 2 years of health care experience • Excellent time management and problem-solving skills • Ability to meet constantly shifting deadlines and timelines • Excellent customer service and recovery skills • Bachelor’s degree in Social Work preferred • Additional related education and/or experience preferred
