📋 Description

• Serve as caregiver to residents during scheduled work periods in long-term care • Provide resident-centered nursing care and daily living assistance under registered nurse supervision • Deliver age-appropriate care according to the care plan • Follow infection prevention and control policies and Personal Protective Equipment use • Assist with bathing, toileting, grooming, dressing, vital signs, psychosocial support, and personal care • Assist residents with transferring, repositioning, walking, range-of-motion exercises, and passive exercises • Complete food preparation and dining tasks • Assist residents with meals and snacks and provide water and supplements • Aid providers and nursing staff with procedures as needed • Document resident interactions and outcomes related to care, behavior, activity, and dietary needs • Communicate changing conditions and care concerns to the charge nurse • Collect non-invasive body fluid specimens and gather vital signs • Provide housekeeping and laundry services • Assist with and document wellness and recreational programs • Encourage residents to participate in programs • Participate in departmental or organizational committees and task forces as needed

🎯 Requirements

• 3 months experience in Long Term Care required (6 months preferred) • Minimum of 18 years of age • Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state certified CNA within four months of date into the position by being enrolled in a training program or in the process of obtaining certification through Endorsement • CNA training and certification required and provided by facility • Satisfactorily completes yearly department specific competency testing • A minimum of 12 hours continuing education are required annually • Maintains all department specific required certifications • When applicable, certified and registered with the Unlicensed Assistive Person (UAP) Registry • Depending on facility, Qualified Medication Administration Person (QMAP) certification with the Colorado Department of Public Health & Environment required • May require a motor vehicle report and proof of valid driver’s license as a Sanford Category II Driver • Must be medically qualified to perform essential driving functions when applicable • Minimum of 30 hours per month • One weekend shift per month • Travel required to support multiple locations within 75-mile radius of permanent address • High school graduate or GED preferred, but not required • Prior clinical or nursing assistant experience in long-term care preferred
