📋 Description

• Ensure members’ safety at home or in community settings • Travel to members’ homes for visits • Manage demands and requests from internal and external stakeholders • Partner with members and families to identify supports and direct services for health, employment, and independent or community living goals • Collaborate with clinical and social support colleagues to address physical, behavioral health, and long-term service needs • Conduct thorough, objective face-to-face visits and assess members’ physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs • Use criteria to authorize appropriate home- and community-based services • Confirm authorized services are provided and members’ needs are met

🎯 Requirements

• 3 years of experience in a clinical setting • 2 years of clinical experience required • Registered nurse with an active license in Tennessee, or a license in the state of residence if participating in the Nurse Licensure Compact Act; or Master's in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW) • Exceptional customer service skills • Must live within Warren or Cannon County, Tennessee • Available for an 8:00am–5:00pm EST schedule • Valid driver's license • TB Skin Test for coordinators working in the field • Basic PC skills, Microsoft Office, and email • Effective time management skills • Excellent oral and written communication skills • Strong interpersonal and organizational skills • Various immunizations and/or associated medical tests may be required • Digital literacy assessment required • Employees routinely operating a company, personal, or rental vehicle must participate in the BCBST Driver Safety Program, maintain a valid driver's license and compliant auto insurance, and maintain an acceptable motor vehicle record
