📋 Description

• Lead the planning, implementation, operationalization, and continuous improvement of enterprise-wide clinical program initiatives • Oversee a portfolio of strategic initiatives across ambulatory, virtual, and specialty care settings • Develop program charters, roadmaps, implementation plans, business cases, budgets, operational models, policies, procedures, specifications, and status reports • Coordinate multidisciplinary teams across clinical operations, population health, specialty care, ambulatory practices, virtual care, digital health, analytics, and IT • Engage stakeholders and lead change-management activities to drive adoption of new workflows and care models • Establish governance structures, milestones, risk mitigation plans, and executive reporting processes • Assess operations and develop implementation strategies for program readiness, compliance, and performance • Identify process-improvement opportunities and recommend corrective actions • Lead product and service go-live events and assess market-specific needs • Coordinate system-wide initiatives with vendors, informatics teams, and frontline service providers • Standardize processes, reporting, and approaches across organizational divisions • Track KPIs and business-to-business operational data and provide interpretation to management at regional, market, and system levels • Support quality-control standards, policy work, remediation of control deficiencies, and monitoring of success metrics • Facilitate teams, meetings, and discussions • Monitor healthcare trends, compliance regulations, standards, and payer directives • Provide feedback to subject matter experts on products and services and suggestions for future program development

🎯 Requirements

• Required: 4-year Bachelor's degree in healthcare, business administration, finance, analytics, informatics, or a related field • Preferred: Graduate/Master's degree in business, healthcare administration, or nursing • Required: 5+ years of experience in healthcare program management, clinical operations, population health, quality improvement, or project management • Preferred: Experience leading complex cross-functional healthcare initiatives • Preferred: Experience with chronic disease management programs or value-based care models • Preferred: Demonstrated experience managing multiple simultaneous projects • Knowledge of value-based care models • Knowledge of healthcare operations and delivery • Experience with EMR systems, preferably EPIC, and integration • Program and project management skills • Ability to develop KPIs, dashboards, and performance improvement plans • Ability to lead through influence and lead multidisciplinary teams • Executive-level stakeholder engagement skills • Change-management skills • Ability to scale operational initiatives across multiple sites of care • Knowledge of Lean principles • Verbal, written, and presentation communication skills • Strong analytical, organizational, and problem-solving skills • Must be located in the continental United States and flexible to work in EST
