📋 Description

• Perform chronic care management services under the supervision of the nurse practitioner for patients participating in Phase 3 Pulmonary Rehab • Encourage patients to reach their health goals while maintaining patient safety • Evaluate medical appropriateness for exercise-based programs • Respond to patients’ emergent medical needs • Maintain a high standard of care aligned with clinical guidelines • Ensure the privacy and accuracy of patient information in compliance with HIPAA regulations • Maintain medical documentation accurately and timely, ensuring proper organization and confidentiality • Conduct biweekly video visits and communicate with patients through phone calls, text, and email • Coach patients on health goals, including accountability, motivation, and encouragement • Monitor vital signs during sessions and handle live emergencies as needed • Coordinate patient care, including making appointments and helping with medication clarifications/adjustments from doctors’ offices • Provide one-to-one education on diseases, therapies, medications, and available patient resources • Provide general administrative support and assist with other tasks to maintain smooth medical-office operations • Work directly with Carda’s founders and clinical team to innovate and improve the digital care delivery model

🎯 Requirements

• Bachelor's degree in Clinical Exercise Physiology / Exercise Science • Minimum of two years of experience in a medical office setting, preferably in cardiac and pulmonary rehab • Proficiency with medical records systems and software • Excellent attention to detail and accuracy in data entry and record management • Knowledge of medical terminology and understanding of medical record documentation • Strong organizational, problem-solving, and communication skills • Familiarity with HIPAA regulations and commitment to patient privacy and confidentiality • Experience in patient care in a clinical setting • Comfortable in emergency situations • Comfortable with live video telehealth visits • Thorough knowledge of common disease processes such as COPD, CAD, and T2DM • Awareness of therapies and medications involved with those diagnoses • Comfortable with various technology platforms and adaptable to fast-paced changes in the clinical setting
