📋 Description

• Facilitate communication among patients/families, multidisciplinary teams, medical management teams, community resources, and other disciplines • Interpret contracts and contractual obligations to support maximum clinical and financial outcomes • Collaborate with patients/families and inter-professional care teams to deliver quality, efficient, and cost-effective healthcare • Work embedded within a provider office or telephonically with a care team • Use evidence-based approaches to increase patient and family activation and engagement • Partner with patients and families to develop SMART goals • Assist with development, procurement, and adoption of patient self-management educational resources • Identify barriers to learning and care delivery; report abnormal findings and collaborate on care plans • Independently manage care-management caseloads and complete required tasks and documentation • Partner with at-risk patients across diagnosis, treatment, and follow-up to provide continuity of care • Coordinate patient information and communication during transitions of care • Coordinate referrals to internal departments and external community resources • Assess patient data and determine priorities based on essential and age-specific needs • Operate necessary equipment and maintain accurate records

🎯 Requirements

• Registered Nurse license issued by the state in which the team member practices • Bachelor's Degree in Nursing or related field • Typically requires 5 years of experience in clinical nursing or 1–2 years of care management experience • Applicable certification is encouraged • Self-directed, with ability to work independently and within a team environment • Excellent oral, written, and interpersonal skills • Critical thinking, problem solving, and organizational skills • Ability to work effectively in a complex environment with multiple changing priorities • Ability to work well with physicians and other healthcare professionals • Proficient computer/Microsoft-suite skills • Previous Epic EMR experience • Ability to handle multiple responsibilities • Ability to provide age-appropriate care and understand principles of growth and development across the life span • Position may require travel between clinic sites
