📋 Description

• Accurately upload and index outside records using applicable software, routing documentation to clinicians as appropriate • Create or select the appropriate patient, encounter, and/or order and assign correct document types and descriptions in the EHR • Incorporate internal and external clinical documentation from electronic and paper sources through work queues, rounding, eDelivery, or importing into the EHR • Meet established quality, accuracy, and productivity benchmarks • Analyze medical records for missing documentation and signatures • Assign and edit medical record deficiencies by responsible providers in the chart management system • Provide support and education to clinicians regarding record completion • Collaborate with Data Integrity and other functional areas on documentation errors and chart correction • Notify leadership for quality review and privacy investigation • Ensure scanned documents are clear, legible, correctly positioned, and readable • Serve as a point of contact for record completion support for clinicians and other providers

🎯 Requirements

• High school graduate or equivalent • Typically requires 0–1 years of experience in healthcare • No licensure, registration, or certification required • Proficient computer and keyboarding skills • Ability to learn Epic, OnBase, Solarity, Microsoft Office, legacy archives, Teams, and electronic communication platforms • High attention to detail and accuracy • Ability to prioritize workload and work under pressure in a fast-paced, time-constrained environment • Ability to work independently and make decisions with minimal supervision while maintaining quality and productivity standards • Ability to work collaboratively in a diverse team environment • Ability to adapt quickly and switch tasks while maintaining quality and accuracy • Ability to receive constructive feedback and take ownership of success • Strong interpersonal and communication skills • Ability to safeguard protected health information and basic knowledge of HIPAA • Ability to operate required equipment, perform repetitive functions and hand movements, and sit most of the workday
