📋 Description

• Accurately upload and index outside records using applicable software, routing documentation to clinicians as appropriate • Create or select the appropriate patient, encounter, and/or order and assign the correct document type and description when indexing into the EHR • Incorporate internal and external clinical documentation from electronic and paper sources through work queues, rounding, eDelivery, or importing into the EHR • Meet established benchmarks for quality, accuracy, and productivity • Apply medical terminology and nomenclature to identify documentation needs based on patient service areas and level of service • Analyze medical records for missing documentation and signatures • Assign and edit medical record deficiencies by the responsible provider in the chart management system according to regulations, bylaws, and policies • Provide support and education to clinicians regarding record completion • Collaborate with Data Integrity and other functional areas on documentation errors and chart correction • Notify leadership for quality review and privacy investigation • Ensure scanned documents are clear, legible, correctly positioned, and readable by end users • Serve as point of contact for record completion support for clinicians and other providers

🎯 Requirements

• High School Graduate or Equivalent • Typically requires 0-1 years of experience in healthcare • Proficient computer and keyboarding skills • Ability to learn Epic, OnBase, Solarity, Microsoft Office, legacy archives, Teams, and electronic communication and meeting platforms • High attention to detail and accuracy • Ability to prioritize workload and work under pressure in a fast-paced environment with time constraints • Ability to work independently and make decisions with minimal supervision while maintaining quality and productivity standards • Ability to work collaboratively in a diverse team environment • Ability to adapt to a fast-paced environment and switch tasks while maintaining quality and accuracy • Ability to receive and apply constructive feedback and take personal ownership for success • Strong interpersonal and communication skills • Ability to safeguard protected health information (PHI) • Basic knowledge of HIPAA • None required for licensure, registration, and/or certification
