📋 Description

• Identify populations, needs, trends, and patterns that may impact outcomes, experience, and cost of care through in-depth analysis • Gather business intelligence and manage research and analysis on social determinants of health, specialized populations, legislation, initiatives, product launches, rules, and regulations • Prioritize strategies that drive social transformation through a comprehensive population health platform and support holistic member care • Collaborate with department leadership to evaluate process-improvement opportunities and measurable benefits • Lead business process and gap analyses and develop reusable solutions using process-excellence principles and related tools • Maintain expertise in federal and state requirements affecting social determinants of health models and recommend standardized population health strategies • Collaborate with Business Development and Integration during RFP processes and implementation of new lines of business • Use data analytics with the Population Health Analytics team to identify improvement opportunities • Examine and explain complex data relationships to answer departmental and cross-functional questions • Ensure operational readiness through initiative oversight and ongoing monitoring of social determinants of health strategies • Work with product and market teams to understand product goals and regulatory guidelines • Review reports and data for patterns, special-cause variation, trends, and associative or causal relationships • Provide management-level summaries explaining key findings across organizational markets and products • Perform other job-related instructions as requested

🎯 Requirements

• Bachelor’s Degree in Social Service, Human Service, Organizational Leadership, or related field or equivalent years of relevant work experience required • Master’s Degree preferred • Minimum of three (3) years of program development, implementation, and evaluation in the social determinants of health or within special populations required • Minimum of one (1) year of experience in data analysis and/or analytic programming required • Healthcare and/or Managed Care experience preferred • Microsoft Suite skills required • Graphic development/presentation skills required • Strategic management skills • Planning, problem identification, and resolution skills • Experience managing large data projects, including meeting deadlines, accuracy requirements, and reporting to upper management required • Excel, Access, and SAS skills • Excellent customer service skills • Ability to work independently and within a team environment • Critical listening, thinking, verbal, and written communication skills • Decision-making/problem-solving skills • Knowledge of government or social investment funding mechanisms required • No licensure or certification required
