📋 Description

• Support care management activities and assigned teams to ensure services are delivered and continuity of care/member satisfaction is achieved • Perform member outreach by phone or home visits • Document care/service plans and related activities • Engage members to discuss care plans, next steps, resources, questions, concerns, and ongoing education • Coordinate care activities with healthcare and community providers, partners, members, and caregivers • Respond to member and provider inquiries, requests, and concerns related to care/service plans • Communicate with care managers, practitioners, and others to facilitate member services and continuity of care • Support service assessments and screenings and document member care needs • Maintain member records in accordance with state and regulatory requirements • Follow contractual, regulatory, and organizational standards and policies • Identify needs and make referrals to Care Managers, community-based organizations, and Disease Managers • Provide education on available benefits and resources • Perform other duties as assigned

🎯 Requirements

• High School diploma or GED required • 1–2 years of related experience required • Ability to perform telephonic and home-visit member outreach • Ability to document care/service plans and member records • Ability to coordinate with healthcare providers, community organizations, members, caregivers, care managers, and practitioners • Ability to identify member needs and make appropriate referrals • Knowledge of contractual, regulatory, and state requirements • Ability to provide education on benefits and available resources • Must live in Arizona
